A CCD camera10picks up multiple adjacent targeted portions on a document2, and to obtain distance information, a distance measurement unit11calculates the distances to the targeted portions of the document2. Based on the distance information, a synthesization controller16prepares perspective information for the images, adjusts the sizes of the multiple obtained plane images so they correspond, and synthesizes the resultant plane images to form a single image.
