ABSTRACT:
Apparatus is disclosed which permits precise location and register of a business form slide image with respect to the alpha-numeric images formed in a computer output microfilmer. The apparatus includes a form slide retainer assembly which receives and holds the form slide. A flexure assembly supports the form slide retainer assembly and enables movement of the form slide in three mutually perpendicular directions, as well as the rotation of the slide, all with enough precision to deliver registered images of both the form and the computer generated data at a film for photographic recording of microfilm records in business form format.
