A template has a flexible sheet with a fixed dimensional array V.sub.1 -V.sub.6 positioned in a specific size configuration appropriate for standard electrocardiographic recording. The distance between V.sub.1 and V.sub.2 is 2.00 inches.+-.0.56 inches, and the distance between V.sub.2 and V.sub.4 is 3.5 inches.+-.1.00 inch, with V.sub.3 located substantially midway between V.sub.2 and V.sub.4, and V.sub.5 is equidistant between V.sub.4 and V.sub.6.
