To form a disk substrate42having a no information area at low cost and without failure, a mold12is provided for forming a disk substrate42having a no information area44. A cavity32of the mold12has a surface50corresponding to the no information area44and made of a mirror-surfaced plate22. A cylindrical member (sleeve35) is inserted in an inner hole38of the mirror-surfaced plate22. An annular projection33is provided on the end surface of the cylindrical member adjacent to the cavity.
